Cardholder Agreement

To access and retain the electronic Communications, you will need the following:

• A computer or mobile device with Internet or mobile connectivity.

• You must have a current version of a PDF reader installed on your device. Common PDF

readers include Adobe Acrobat Reader, Foxit Reader, and other similar programs.

• For desktop website-based Communications:

o Recent web browser that includes 256-bit encryption;

o The browser must have cookies enabled. Use of browser extensions may impair full

website functionality; and

o Minimum recommended browser standards are the most recent versions of Mozilla

Firefox (see http://www.mozilla.com for latest version), Apple Safari (see

http://www.apple.com/safari for latest version), or Google Chrome (see

http://www.google.com/chrome for latest version).

• For application-based Communications:

o A recent device operating system that supports text messaging, downloading, and

applications from the Apple App Store or Google Play store; and

o The most recent versions of Apple Safari or Google Chrome on iOS or Google Chrome

for Android OS.

As a cardholder of the QWIK Card prepaid card issued by Epay Card Programs INC,

hereby acknowledge and consent to the use of electronic signatures and records for all

transactions related to my prepaid card.

I understand that QWIK by Epay has an established E-Sign Act Policy. In accordance with

this policy, I acknowledge that communications, agreements, statements, notices,

disclosures, or other documents related to my prepaid card may be provided electronically rather than in paper form. I further understand that these electronic documents will carry the same legal weight and effect as if they were provided in paper format.

Moreover, I am aware of my right to request paper copies of any electronic document related to my prepaid card, by sending an email to support@qwikcard.app I understand that I may request these copies at any time incurring additional costs.

The cost of providing physical copies of electronic records will be determined by the number of printed pages required by the Cardholder. The total number of printed pages will depend on the volume of transaction activity, with multiple transactions potentially fitting on a single page. Each printed page will incur a fee of $2.00 USD, which includes all shipping and handling charges.

Should I choose to withdraw my consent for electronic transactions at any point, I can do so by sending an email to support@qwikcard.app I understand that by withdrawing my consent, QWIK by Epay will continue to provide the service to me without any extra fee except for the cost described above for each sheet printed.
